“ baba Vanessa siwezi kusema chochote wala lolote mpaka na wewe ukapime “

“ sikia mama Vanessa mimi siendi kupima sina ukimwi kwani umeniona nikiumwa ?”
“ baba Vanessa , wewe huwa unasafiri sana maeneo mengi na sijui kuhusu afya yako kwa muda mrefu sasa mbona hivyo leo ?”
“ aaaaaahhh , baba Vanessa utaniua “

Diasaster Preparedness

DISASTER PREPAREDNESS

By Dr. Amur Abdullah Amur
Principal Specialist in Internal Medicine, Muhimbili National Hospital, Dar-Es-Salaam

The success of the plan of action to rescue miners in Chile reflects to well organized disaster preparedness activities by the Public Health Authorities. This rescue involved Chilean miners who had been trapped for almost 70 days some 600 meters underground after the mine they were working in collapsed around them 5th Aug.2010 , puzzled the attention of many.

The possibility of Public Health Emergencies concerns many people in the wake of frequent outbreaks of diseases and environmental mishap particularly in developing countries.

Many would agree with me that disaster preparedness helps people to deal with disasters of all sorts much more effectively when they do occur. Disaster preparedness is efficiently accomplished from family level to the national level. Charity starts at home! At a family level, keeping a three-day supply in the reserve and at least an additional four-day supply per person for use at home by each family is advisable. Public Health Disaster Preparedness teams are expected to provide operational requirements include tents and toilet facilities, contracted support for food, potable water, protective gears, transport , portable medical kits including oxygen cylinders, and biomedical waste disposal. Disaster Teams provide primary and acute care, initial resuscitation and stabilization, advanced life support and preparation of sick or injured for evacuation. Each team includes physicians, nurses, medical technicians, and supporting staff. The team is expected to sustain operations for 72 hours without external support.

Disasters disrupt hundreds of thousands of lives every year and have lasting effects, both to people and property. There is a need to establish tailor made Training Courses on Disaster Management that involve all walks of Life. During our school time, we used to have Scout Clubs that conduct elementary disaster preparedness programs. Such activities should be incorporated formally to an accredited level so as to produce cadres who can serve as Emergency Medical Responders, Paramedics and Members of Ambulance Teams. Institutes like Red Cross and Red Crescent play vital role in conducting short and long term First Aid Training. Provision of assistance consists of medical materiel, personnel, and technical assistance. These resources may provide response capability for the emergency response needs. Collaboration in advance with rescue teams helps to promote alertness, disseminate health information, minimize confusion, and identify appropriate actions and responses.

Due to increasing vulnerability of developing countries to suffer various natural disaster in a form of outbreak of infectious diseases and environmental hazards, there is a need to establish Disaster Management Institute within the Public Health Program.

Lesson from Chile has proved that appropriate disaster preparedness reduces the devastating impact on human life, economy and environment . This is achieved through education, implementing warning strategies , toll free numbers, safety measures, users friendly infrastructures, developmental plans to provide coverage to such disasters ,provision of resources and rehabilitation as well as post-disaster reconstruction.

Disaster Management is multidisciplinary approach which involves nationwide participation, different professions and different scientific fields, and has become an important measure for human, society and nature sustainable development. Developing countries that lack the necessary facilities, planning, expertise and resources for

disaster preparedness arguably need them the most. The development of Disaster Preparedness within a Public Health Program should be seen as an important opportunity for the development within the nationwide health program.

The responsibility of Disaster Preparedness is to provide for a comprehensive Emergency Management Program that meets the emergency needs of those who either have been or might be affected by an emergency or major disaster for example cholera outbreak .

The population explosion and exposure to inadequate space, substandard sanitation, pollution, poor housing and lack of safety measures in poor resource countries have increased the frequency and severity of disasters. Surface decontamination remains an uphill task in case of chemical or biological pollution in developing countries. With the prevailing background of the high disease burden an environmental hazards present in many poor resource countries, the cost of running Disaster Management could be very high. In any public health program, a well-integrated Disaster Preparedness ultimately result in cost savings through early recognition and management of the disasters.

Some countries have units of Medical Response Cops(MRC) consist of organized medical and public health professionals who serve as volunteers to respond to natural disasters and emergencies. The MRC helps to engage medical and public health personnel in response to an emergency. MRC team is capable of providing primary and secondary medical care as well as to stabilize victims for transportation to hospital facilities .

Application of proper disaster management strategies involves formation of disaster management teams ,identifying phases of disaster preparedness, pre-disaster planning, resources preparedness, monitoring including relief management capability, prediction, early warning, damage assessment and relief management. Disaster Preparedness aims to save lives, prevent injuries, preserve the environment and protect property and the economy. Disaster management is comprised of four interdependent risk-based functions: prevention/mitigation, preparedness, response and recovery. Mitigation provides a critical foundation for disaster management.

The followings are the Phases of Disaster Preparedness:

1.Mitigation/Prevention -- A period of time during which activities are undertaken by individuals/institutes to improve their capabilities to respond to a potential emergency and fulfill their assigned responsibilities. Mitigation efforts attempt to prevent hazards from developing into disasters altogether, or to reduce the effects of disasters when they occur. The mitigation phase concentrates on short and long-term measures for reducing or eliminating risk. The implementation of mitigation strategies can be considered as a part of the recovery process if applied after a disaster occurs. The need of Establishment of Curriculum to train cadres of Disaster Management Team should be emphasized. Strategies of mitigation eliminate or reduce the impacts and risks of hazards through proactive measures taken before an emergency or disaster occurs.

2. Preparedness --

A period of time during which activities are undertaken by individuals and disaster teams to increase their alertness during periods of heightened risk. The emergency managers develop plans of action to manage and counter their risks and take action to build the necessary capabilities needed to implement such plans during this phase. Common preparedness measures include communication plans, emergency population alertness, evacuation, shelters, and provision of supplies. Public Health Program have the task to ensure first responders and emergency management personnel are well-prepared through education, support and exercises.

3. Response --

A period of time during which activities are undertaken by individuals and institutes to respond to an occurrence that threatens or harms human beings and environment. The response phase includes the mobilization of the necessary emergency service and members of disaster teams in the affected area. A well organized emergency plan activities developed as part of the preparedness phase enables efficient coordination in saving life ,prevention of injuries and protection of environment . The earlier response commences the better the outcome . Depending on impact of disaster sustained by the victims, virulence in case of infections, and access to health management, the vast majority of those affected by a disaster will die within 72 hours after impact. Regardless of the cause of the disaster, hygienic precautions such as hand washing ,wearing protective gears must be the rule of the game. Thorough hand-washing with antiseptics between examining victims and after contact with blood, body fluids, secretions, excretions, and equipment contaminated by them is an important component of infection control during disaster.

4. Recovery --

A period of time during which activities are undertaken by individuals and institutes to provide for the welfare of the people following a Disaster and/or emergency. Recovery efforts are primarily concerned with actions that involve counseling, crisis intervention, rehabilitation , moral support and rebuilding destroyed property. Efforts should be made to build user friendly structures taking safety measure techniques into account . This requires collective responsibilities and commitment of the community and the nation to develop and deliver disaster financial assistance, determining the amounts and types of assistance that will be provided to those who have experienced losses.

What Has Juma Been Up To?

You may remember him from the movie Bongoland as Juma, the movie that put him on the world map. He is a self created artist with a solid inspirational vision for his future.

His real name is Mukama Morandi. He stands at 5’11 and weighs close to 180 pounds on a good day. When you meet Mukama, you automatically think “this guy is somebody”.

It is difficult to describe, but he has an overwhelming presence that makes you do a double take each time you see him. Maybe it is his height but then again maybe it is his smirk smile. His stature was good enough to win him a leading role in Bongoland.

Gavana Mweusi wa Massachusetts Ashinda Uchaguzi!

Gavana wetu, Mh. Chekeni!

Heaven, Hell na Wanasiasa

Hii inafaa kwa wanasiasa wote duniani kote!!!!

*******************************

HEAVEN AND HELL

While walking down the street one day a Corrupt Senator was tragically hit by a car and died.

His soul arrives in heaven and is met by St. Peter at the entrance.

"Welcome to heaven," says St. Peter. "Before you settle in, it seems there is a problem. We seldom see a high official around these parts, you see, so we're not sure what to do with you."

"No problem, just let me in," says the Senator..

"Well, I'd like to, but I have orders from the higher ups. What we'll do is have you spend one day in hell and one in heaven. Then you can choose where to spend eternity."

"Really?, I've made up my mind. I want to be in heaven," says the Senator.

"I'm sorry, but we have our rules."

And with that, St. Peter escorts him to the elevator and he goes down, down, down to hell.

The doors open and he finds himself in the middle of a green golf course. In the distance is a clubhouse and standing in front of it are all his friends and other politicians who had worked with him.

Everyone is very happy and in evening dress. They run to greet him, shake his hand, and reminisce about the good times they had while getting rich at the expense of the people. They played a friendly game of golf and then dine on lobster, caviar and the finest champagne.

Also present is the devil, who really is a very friendly guy who is having a good time dancing and telling jokes.

They are all having such a good time that before the Senator realizes it, it is time to go.

Everyone gives him a hearty farewell and waves while the elevator rises...

The elevator goes up, up, up and the door reopens in heaven where St. Peter is waiting for him, "Now it's time to visit heaven.."

So, 24 hours passed with the Senator joining a group of contented souls moving from cloud to cloud, playing the harp and singing. They have a good time and, before he realizes it, the 24 hours have gone by and St. Peter returns.

"Well, then, you've spent a day in hell and another in heaven. Now choose your eternity."

The Senator reflects for a minute, then he answers: "Well, I would never have said it before, I mean heaven has been delightful, but I think I would be better off in hell."

So St. Peter escorts him to the elevator and he goes down, down, down to hell..

Now the doors of the elevator open and he's in the middle of a barren land covered with waste and garbage. He sees all his friends, dressed in rags, picking up the trash and putting it in black bags as more trash falls from above.

The devil comes over to him and puts his arm around his shoulders.

"I don't understand," stammers the Senator. "Yesterday I was here and there was a golf course and clubhouse, and we ate lobster and caviar, drank champagne, and danced and had a great time. Now there's just a wasteland full of garbage and my friends look miserable. What happened?"

The devil smiles at him and says,

"Yesterday we were campaigning, Today, you voted.."

Anti-Obama billboard taken down

GRAND JUNCTION (Colorado) - A BILLBOARD in western Colorado that used caricatures to depict President Barack Obama as a terrorist, gangster, Mexican bandit and a gay man has been taken down.

The sign along an Interstate 70 business loop in Grand Junction drew criticism from local Republicans and Democrats. The artist, Paul Snover, of nearby Loma, says he has received what he calls 'very hateful' e-mails and phone calls.

Mr Snover has said he couldn't reveal who paid for the billboard.

The Daily Sentinel said he couldn't be reached on Friday to explain why it was removed.

The owner of a company where the sign was erected says she's glad it's gone because it attracted such a crowd. -- AP

Oprah Asikitika Matron wa Shule Yake Ameachiwa Huru!

Talk Show Mogul Oprah “Disappointed” At Sexual Assault Trial Outcome

Los angeles 10/12/2010

Talk show Queen Oprah Winfrey has said she is "profoundly disappointed" at the acquittal of a woman accused of sexual assault at the talk show hosts South African school for teenagers.

Tiny Virginia Makopo, the former school matron, was accused by the prosecution of 14 counts of sexual assault, including trying to fondle and kiss the girl students at the school. She was also charged with assaulting one of the pupils and a fellow supervisor at the college.

Prosecutor Etienne Venter has after the trial, "She was found not guilty on all of the charges," and according to Sapa news agency, Venter would not be seeking an appeal against the court’s ruling.

South Africa's National Prosecuting Authority spokesman Mthunzi Mhaga commented that: "The magistrate indicated that the state did not prove itself case beyond reasonable doubt on all the charges. We won't be appealing the judgment."

The allegations first arose around the matron at Oprah’s $40 million school, or “Leadership Academy” soon after it opened just outside South African capital Johannesburg in 2007.

Oprah has said that she was "shaken to the core" by the scandal, and the claims of sexual abuse at a place she had thought would be a refuge. She said that it was one of the most devastating periods in her life.

She was recorded Monday saying that the staff at her school are "committed to providing a nurturing educational environment so that all of our girls may continue to flourish. And they are indeed thriving".

Spike Lee Boston University Leo

US apologizes for infecting Guatemalans with STDs in the 1940s

October 1, 2010

Washington (CNN) -- The United States apologized Friday for a 1946-1948 research study in which people in Guatemala were intentionally infected with sexually transmitted diseases.

A statement by Secretary of State Hillary Clinton and Secretary of Health and Human Services Secretary Kathleen Sebelius called the action "reprehensible."

"We deeply regret that it happened, and we apologize to all the individuals who were affected by such abhorrent research practices," the joint statement said. "The conduct exhibited during the study does not represent the values of the United States, or our commitment to human dignity and great respect for the people of Guatemala."

President Barack Obama called his Guatemalan counterpart Friday "offering profound apologies and asking pardon for the deeds of the 1940s," President Alvaro Colom told CNN en Espanol in a telephone interview from Guatemala City.

"Though it happened 64 years ago, it really is a profound violation of human rights," said Colom, who said the report took him by surprise.

An international, independent commission will carry out an investigation, he said.

Clinton called him on Thursday, he said. "She too offered her apologies," he said, adding that she told him she was ashamed the United States had been involved in the matter.

Asked whether Guatemala was planning to take legal action, Colom said, "That's part of the work of the comission."

"We reject these types of actions, obviously," said Guatemala presidential spokesman Ronaldo Robles. "We know that this took place some time ago, but this is unacceptable and we recognize the apology from Secretary Clinton."

The study came to light recently when Wellesley College researcher Susan Reverby found the archived but unpublished notes from the project.

The scientific investigation, called the U.S. Public Health Service Sexually Transmitted Disease Inoculation Study of 1946-1948, aimed to gauge the effectiveness of penicillin to treat syphilis, gonorrhea and chancres. Penicillin was a relatively new drug at the time.

The tests were carried out on female commercial sex workers, prisoners in the national penitentiary, patients in the national mental hospital and soldiers. According to the study, more than 1,600 people were infected: 696 with syphilis, 772 with gonorrhea and 142 with chancres.

A similar study was conducted between 1932 and 1972 in Tuskegee, Alabama, on nearly 400 poor African-American men with syphilis whose disease was allowed to progress without treatment. The subjects were not told they were ill with the disease.

The Guatemala study was done under the direction of U.S. Public Health Service physician John C. Cutler, who later ran the Tuskegee experiment, said Dr. Francis Collins, director of the U.S. National Institutes for Health.

Collins told reporters that the Guatemala study represented "a dark chapter in the history of medicine."

U.S. officials said Friday that ethical safeguards would prevent such abuses from occurring today.

"The study is a sad reminder that adequate human subject safeguards did not exist a half-century ago," the U.S. statement said. "Today, the regulations that govern U.S.-funded human medical research prohibit these kinds of appalling violations."

Clinton and Sebelius said the United States is launching an investigation and also convening a group of international experts to review and report on the most effective methods to make sure all human medical research worldwide meets rigorous ethical standards.

"As we move forward to better understand this appalling event, we reaffirm the importance of our relationship with Guatemala, and our respect for the Guatemalan people, as well as our commitment to the highest standards of ethics in medical research," the U.S. statement said.

Josiah Kibira University in Bukoba

This summer, president Jakaya Kikwete was in Bukoba to attend the 100 anniversary of the Lutheran Church in the area. As part of those festivities, Josiah Kibira University College was dedicated.

In his speech, Kikwete noted that it was about time for Bukoba to have a university since Bukoba people are known to be "Nshomile" (Booksmart) and yet had no university in the area. The comment was all in good fun, the crowd reciprocated with a weak cheer and laughter.

Bedbug Outbreak: Grim Summit on How to Fight Infestation

It's possible that bedbugs are simply the latest in a long string of public panics over epidemics that eventually proved to be manageable, from killer bees to bird flu. But that would not quite capture the grim mood of a big bedbug summit, held the past two days at a hotel near Chicago's O'Hare International Airport. More than 350 people gathered at the two-day event, which sold out five weeks in advance.

One scientist showed, using DNA evidence, how the bugs hop blithely from continent to continent on clothing, carry-ons, backpacks and purses. Another tracked the eruption of bedbug colonies through a single multistory apartment building. Another flashed a photograph of a robust bedbug literally glistening with drops of poison rolling off its back. (See pictures of bedbugs.)

Judging by the number of note-taking exterminators at the summit, however, even pest-control professionals are still searching for tips. There is no perfect insecticide. Even if DDT were magically restored to the market, bedbugs have evolved a resistance to their once mortal foe. Many professionals advise clients to zip their mattresses and box springs into impermeable casings, but not all products are created equal. Summit organizer Phillip Cooper says he has seen hungry bugs squeeze between the zipper teeth of ineffective mattress cases.

The exhibition hall in Chicago suggested the range of bedbug-fighting strategies. Mobile ovens offered to bake the bugs to death. ("Heat is the bedbug's Achilles' heel," says White.) Powerful vacuums proposed to suck them to oblivion. Some experts favored steaming; some favored poison dust; some suggested electrocution. A woman from Australia claimed to have eliminated her infestation by installing simple barriers on her bed legs and waiting patiently for the bugs to starve. (See pictures of the fascinating, frightening world of insects.)

A better solution, experts agree, is to avoid importing bedbugs in the first place. Be cautious in public places where upholstered or wooden surfaces meet backpacks and purses - movie theaters, public libraries, even changing tables in public restrooms. Travelers should inspect hotel beds and headboards for signs of bedbugs; keep suitcases off the bed; unpack outside upon returning home; and put travel clothes immediately into a hot-water wash. At least one vendor offers plastic bags that dissolve in the laundry, so that travel clothes can be sealed at the hotel and dropped directly into the hot-water cycle. If you really want to be thorough, you can buy a $330 PackTite oven designed to bake your luggage to a bug-killing temperature. Otherwise, it might help to seal your suitcase in a plastic garbage bag.

The good news about bedbugs is that they don't appear to transmit diseases. And summit organizers think awareness and innovation should eventually make bedbugs a manageable problem for most people. "Mark my words: In the next 10 years, a silver bullet will be found," says organizer Phillip Cooper. "Somebody will figure out how to deal with bedbugs, and after that, it will be just another pest, like roaches or yellow jackets."

Morgan Freeman, Missus Finalize Divorce

Look out, ladies, Morgan Freeman is officially a free man.

A Mississippi judge has finalized the divorce of the Academy Award-winning actor from his second wife of 26 years, Myrna Colley-Lee.

Freeman's Jackson-based lawyer, William R. Wright, tells E! News that a divorce decree was issued on Wednesday, though terms were kept under seal.

"There's no waiting period, there was no trial and they reached an amicable settlement settlement," said the legal eagle. "He is just happy that they can both move on."

The Dark Knight star married Colley-Lee, a theater and film costume designer, on June 16, 1984, and the couple were together during Freeman's rise to the top of the film world as one of Hollywood's most durable and acclaimed actors.

In August 2008, however, the thesp's business manager announced the pair had filed for divorce after separating the previous December. No reason was given for the split, but the divorce petition came just days after Freeman was involved in a scary car crash with a female friend of the family's.

Freeman, 73, was previously hitched to Jeanette Adair Bradshaw from 1967 to 1979.

Nigerian advance-fee scammer gets 12 years

Over five years, the man racked up more than $1.3 million

A Nigerian man has been sentenced to 12 years in prison for sending out fraudulent e-mails offering victims big bucks in exchange for moving cash to the United States.

Okpako Mike Diamreyan, 31, was sentenced to 151 months of prison Wednesday by United States District Judge Janet Hall in Bridgeport, Connecticut.

Diamreyan made more than US$1.3 million in a scam that suckered 67 victims between 2004 to 2009, prosecutors said. This type of fraud, called an advance-fee scam, was the number-one type of Internet fraud in 2009, according to the U.S. Federal Bureau of Investigation. Last year, advance-fee fraud accounted for nearly 17 percent of the Internet fraud logged by the FBI.

Diamreyan pretended to be different people -- Prince Nana Kamokai of Sierra Leone or an airport director from Ghana, for example. He said he needed to move between $11.5 million and $23.4 million out of the country and offered victims 20 percent of the funds, if they would help him out.

After using fake documentation to convince his victims that he was legitimate, Diamreyan would get them to wire him different types of fees such as "PIN code fees" or courier services charges with the understanding that they would then get the money. These fees would pile up, but the promised money never arrived.

The scam left "many individuals and their families in financial ruin," the U.S. Department of Justice said in a statement.

Diamreyan immigrated to the United States in 2008 and allegedly told an acquaintance that he wanted to make $1 million or so before going home to Nigeria: "i want to forget america and come back home... once i take like 1m or half m," prosecutors quote him as saying.

He was arrested in August 2009 and found guilty by a jury in February.
