Captain John Komba Afariki Dunia

 Wadau, Mh. Captain John Komba, Mbunge wa Mbinga amefariki leo katika hospitali ya TMJ. Wanasema alikuwa anasumbuliwa na kisukari (diabetes).

Lazima niseme kuwa nakumbuka marehemu Captain Komba alivyoaanza kupanda chati. Alikuwa Sargent wa Jeshi na alikuwa anaimba katika kikundi cha wasanii. Alimwimbia Mwalimu ule wimbo wa 'We Love Freedom'.  Nimesahau ilikuwa sherehe ya nini. Mwalimu na wote tuliyokwepo tuliblow!  Alipata U-Captain haraka sana na mambo ya TOT ni ya kihistoria.

Baada ya kupata taarifa hizo, paparazi wetu alitinga katika zahanati hiyo ili kuhakikisha kama kweli mama huyo alikuwa amelazwa, alipofika nje aliliona gari la marehemu Kanumba na alipoingia ndani alimuona mama huyo.
Hata hivyo, jitahada za kupata picha yake akiwa kitandani zilishindikana kutokana na mazingira kutoruhusu.
Aidha, Ijumaa liliongea na mdogo wa marehemu Kanumba, Seth Bosco ambaye alikiri mama yake kulazwa katika zahanati hiyo.

Annexure – B

1. Personal Particulars:

You will keep us informed of any change in your residential address, your family status or any other relevant particulars. You would also let us know the name and address of your legal heir/nominee.

2. Nature of Work:

You will partake in our Five (5) days Training, Lecturing and Orientation Workshop organized by the Management of West Park Hospital/University for our new employees, to give you the opportunity to know everything relating to your duties and the Institution during your stay here in USA.

3. Working Hours:

The regular working hours of the University are from (7:00am to 4:00pm Monday to Friday) including 50 minutes for lunch break and 30 minutes for tea break.

4. Late comings:

The University follows a strict time schedule and late comings are discouraged, unless otherwise notified by you in advance. Late marks will be accorded to you for every late entry with one day marked as absent for every three late marks.

5. Assignment, Transfer and Deputation:

Though you have been engaged to a specific position, the University reserves the right to send you on deputation/transfer/assignment to any of the University’s branch offices within the country or abroad, whether existing at the time of your appointment or to be set up in the future.

6. Training:

You will hold yourself in readiness for any training at any place whenever required. Such training would be imparted to you at the University’s expense. Kindly note that refusal to participate in a training programme without any extraneous circumstances would lead to automatic termination of your employment.

7. Performance Reviews/Appraisal:

The University follows a policy of quarterly and annual performance reviews, which are linked to performance incentives every year. You will be eligible for performance based incentives on the basis of your gross monthly salary on the basis of your performance during the previous quarter/year.

The University also has a policy of six monthly/annual salary appraisal based on performance. You will be eligible for a salary appraisal under this policy based solely on the performance ratings achieved during the previous six months/one year.

8. Intellectual Property Right:

If during the period of your employment with us you achieve any invention, process improvement, operational improvement, or other process/method likely to result in more efficient operation of any of the activities of the University, the University shall be entitled to use, utilize and exploit such improvement and you shall assign all rights thereof to the University for the purpose of seeking any patent rights or for any other purpose. The University shall have the sole ownership rights of all the intellectual property rights that you may create during the tenure of association with the University including but not limited to the creative concept that you may develop during your association with the University.

9. Secrecy/Confidentiality:

You will not during the course of your employment with the University or at any time there after divulge or disclose to any person whomsoever including competitors and former employees, make any use whatsoever for your own purpose or for any other purpose other than that of the University, of any information or knowledge obtained by you during your employment as to the business or affairs of the University including development, process reports and reporting system and you will during the course of your employment hereunder also use your best endeavor to prevent any other person from doing so. Failure to do so on your part may result in legal action against you and the person to whom the information was divulged.

10. Restrain:

i. Access to Information:

Information is available on need to know basis for specific groups and the information is segregated to allow individual sectors information access for projects and units. Access to this is authorized through access privileges approved by unit mentors or project mentors. Unauthorized access or attempt at unauthorized access is strictly prohibited and any attempts to do so will result in immediate termination of employment and legal action as deemed fit by the University.

ii. Restriction on Personal Use:

Use of University resources for personal use is strictly restricted. This includes usage of computer resources, information, internet service, and working time of the University for any personal use. You will under no circumstances carry any work home unless specifically requested by your manager. Any usage of University information for personal use will result in immediate termination of employment without notice and/or legal action for misdemeanor as deemed fit by the University. You may/may not be required to reimburse the University for any losses incurred by the University on account of personal usage of University data.

11. Leave:

You will be entitled to leave as per law in force and as laid down in the Standing Orders of the University. The University follows strict time schedule and late comings are discouraged, unless otherwise notified by you in advance. Late marks will be accorded to you for every late entry with one day of absence counted for every three late marks.

12. Security:

Security is an important aspect of our communication and office infrastructure. Communication security is maintained by controlling physical access to computer system, disabling all working stations, floppy disk drives and Universitywide awareness about the need for protection of intellectual property and sensitive customer information.

13. Termination of Service:

i. Either party can terminate this employment by serving a notice of one month on the other, save and accept that the University may at its option pay salary in lieu of the notice period to terminate employment with immediate effect.

ii. Unauthorized absence or absence without permission from duty for a continuous period of 7 days would make you loose your lien on employment. In such case your employment shall automatically come to an end without any notice of termination or notice pay.

iii. You will be governed by the laid down code of conduct of the University and if there is any breach of the same or non conformance of contractual obligation or with the terms and conditions laid down in this agreement, your service can be terminated without any notice; notwithstanding any other terms and conditions stipulated herein the University reserves the right to invoke other legal remedies as it deems fit to protect its legitimate interest.

14. Standing Orders:

You will abide by the Standing Orders, rules & regulations and service conditions that may be in force or application to the organization or are framed from time to time by the University.

15. Appointment in Good Faith:

It must be specifically understood that this offer is made based on your proficiency on technical/professional skills you have declared to possess as per your application for employment and your ability to handle any assignment/job independently. In case at a latter date any of your statements/particulars furnished are found to be false or misleading or your performance is not up to the mark or falls short of the minimum standard set by the University, the University shall have the right to terminate your services forthwith without giving any notice notwithstanding any other terms and conditions stipulated therein.

The above terms and conditions are based on the University’s policy, procedures and other rules currently applicable in the country and are subject to amendments and adjustments from time to time. In all matter including those not specifically covered here such as traveling, retirement, etc. you will be governed by the rules of the University as shall be in force from time to time.

[Updated at 10:54 a.m. ET] House Speaker John Boehner renewed calls to reverse the Obama administration's signature health care overhaul after Thursday's Supreme Court decision upholding the act, saying the decision "underscores the urgency of repealing this harmful law in its entirety."

[Updated at 10:52 a.m. ET] House minority leader Nancy Pelosi declared the court's ruling a "victory for the American people."

"With this ruling, Americans will benefit from critical patient protections, lower costs for the middle class, more coverage for families, and greater accountability for the insurance industry," she said.

Pelosi further said that the Affordable Healthcare Act will prevent children from being denied coverage for pre-existing conditions, lower drug costs for seniors and allow students and young adults to stay on their parents’ plan.

“In passing health reform, we made history for our nation and progress for the American people," Pelosi said. "We completed the unfinished business of our society and strengthened the character of our country. We ensured health care would be a right for all, not a privilege for the few."

It’s the numbers that fascinate Dr. Godwin Godfrey, a 32-year-old Tanzanian who is finishing a five-year training course in Israel to become a pediatric cardiac surgeon.

When he graduates next summer, he will become the first and only pediatric heart surgeon in all of Tanzania – a country of 43 million people.

Tanzania’s Ministry of Health already has a waiting list of nearly 500,000 patients who desperately need his services.

The government, which sends most of its patients who need heart surgery overseas to countries like India, can only afford to care for about 100 people a year.

“You can see the chances of making it are slim,” says the surgeon. “A child on the waiting list with a serious heart condition may wait maybe three or four years and die of complications like heart failure.

“Sometimes it can be very depressing being a doctor in a poor country like Tanzania,” he says. “Too many times you see children dying in front of your eyes. You know that you are supposed to do something, like give them medicine, but you don’t have it.”

When he first started his training as a pediatric surgeon in Israel’s Wolfson Medical Centre under a program sponsored by the Save A Child’s Heart charity, Dr. Godfrey said he experienced something akin to “culture shock.”

“I always used to see a lot of death,” he says. “In our department in Tanzania [in the Bugando Medical Center in the city of Mwanza — an 850-bed hospital that serves 15 million people] you could have five or seven children die every day. When I got to Israel to train I realized we weren’t seeing any children die — even in a month.

“This was really an eye-opener. Whatever we were doing; it was wrong,” he says. “It opened my eyes. There is so much that we don’t know. There is so much equipment that we don’t have. There is so much medication that we don’t have.”

‘I always used to see a lot of death’

The son of a surgeon, Dr. Godfrey, who was born in a small town on the slopes of Mount Kilimanjaro, has been familiar most of his life with the difficulties of practicing medicine in a developing country.

After graduating at the top of his class in high school in Tanzania, he won a scholarship to study medicine at Makerere University in neighbouring Uganda. When he returned home to serve his internship, he deliberately selected the Bugando Medical Centre in Mwanza, Tanzania.

“Most of our doctors are actually located in Dar es Salaam, the capital,” he says. “That’s the biggest city and all the government offices are there. But with me being interested in surgery, I thought if I went to work in one of the big hospitals I wouldn’t get the hands-on experience that I wanted.”

In Mwanza, he was overwhelmed with work and because of a shortage of surgeons and specialists found himself doing everything from pediatric cases to orthopedic, ophthalmology and neuro-surgery.

When he started a residency in the Mwanza hospital, he shifted to cardiac surgery, but found himself stymied by a lack of equipment and expertise.

‘Being in Africa, in this hospital, we didn’t have the equipment to conduct open-heart surgery’

“Being in Africa, in this hospital, we didn’t have the equipment to conduct open-heart surgery,” he says. “So we have had to start building our unit from scratch.”

When Dr. Godfrey investigated the possibility of getting some expert training in cardiac surgery, he discovered the only place he could do so in Africa was in South Africa, Egypt, Morocco or Tunisia. But when he tried to apply, he was told those countries weren’t interested in training foreign doctors.

Then in 2006 a visiting German doctor, working for a Christian charity, suggested Dr. Godfrey should apply to study in Israel under a humanitarian project sponsored by the Save A Child’s Heart organization.

Over the last 15 years, cardiac surgeons operating out of the Wolfson Medical Centre in Holon, Israel, have treated more than 2,800 children with congenital heart diseases and adolescents with heart valve diseases.

Patients are now flown into Israel from all over the world for about 225 emergency operations each year, with 43% of the cases coming from Africa and 47% from neighbouring Arab states.

Save a Child’s Heart offered Dr. Godfrey a full five-year scholarship to study pediatric cardiac surgery and has methodically begun to help him train a complete surgery team to work with him in Tanzania.

“Heart surgery is teamwork,” says Dr. Godfrey, who was in Toronto this week to tour Sick Children’s Hospital. “It’s not just the work of a surgeon. You need a cardiologist, an anesthesiologist, an intensive care unit internist, people to operate the heart pumps and ventilators.”

‘We had to stop her heart, go in and close this hole and then start the heart functioning again’

Last summer, a team of 13 doctors from Save a Child’s Heart travelled to Tanzania to stage a cardiac clinic in Dr. Godfrey’s hospital and operated on 13 seriously ill children, including the first ever open-heart operation on a child in Tanzania.

Four year-old Laurencia Simon, the daughter of farmers who live in a mud hut without electricity or running water three hours drive from Mwanza, underwent the historic surgery.

“She had a hole in her heart,” Dr. Godfrey explains. “We had to stop her heart, go in and close this hole and then start the heart functioning again.”

“In places like Canada, you can do this operation any day,” he says “You don’t let patients with the condition become six months old or even a year, without the operation. But in Tanzania, this little girl was four years old and she could have died while being on a waiting list.”

Now, she is healthy and back in school.

In a very small way, things are about to change in Tanzania.

Dr. Mbele Asifia Tiba Aliyopata Dar

Wadau, Prof. Mbele yuko safarini Tanzania.  Ameugua na kwenda hospitalini. Anasema kuwa ameridhika na tiba aliyopata, hakuwa na haja ya kukimbilia nchi za nje kam India kutibiwa.

Nimeugua Dar es Salaam; Natibiwa Hapa Hapa, Sio India

Nimeugua Dar kuanzia siku chache zilizopita. Nimejikuta nikiwashwa hasa mikononi, miguuni, na mgongoni. Kwa siku mbili sikuweza kulala usiku. Katika kuulizia watu, nikaambiwa ni "allergy." Na mhudumu mmoja kwenye duka la madawa maeneo ya Sinza aliniambia niende Magomeni, kwenye hospitali ya Dr. Ole.

Nilienda hapo. Wakati wa kuchukuliwa damu ili ipimwe, mpimaji alinipiga mchapo kuwa isije ikawa nimewekewa sumu ya unga. Akazidi kutonya, huku akijifanya ananyunyizia unga mezani, kuwa kaunga kidogo tu kanatosha. Nami nilijibu kuwa mtu huwezi kujua, kwani Bongo haina dogo. Tukawa tunachkea.

Ni kweli daktari alipochunguza ripoti ya upimaji amegundua nina "allergy," na akanielezea sababu zake. Aliniandikia sindano na vidonge vya aina mbali mbali. Alisema kuwa hiyo dozi itafuta kabisa hili tatizo. Nimedundwa sindano jana na vidonge nikaanza. Tayari nimeshaanza kujisikia nafuu. Naendelea vizuri kabisa.

Nimeleta habari hii binafsi kwa sababu maalum. Kwanza, mimi kama m-Tanzania nina imani na madaktari wetu. Ninafundisha Marekani, na ninapougua kule natibiwa kule. Ninapougua Tanzania natibiwa hapa. Tena, miaka yote ya maisha yangu, kabla sijaenda Marekani nilikuwa natibiwa hapa Tanzania, bila matatizo.

Sioni kama kuna tofauti kati ya madaktari wa Marekani na wa kwetu. Kilichopo tu ni kuwa madaktari wa kwetu wapewe mahitaji yanayopasika katika kumshughulikia mgonjwa. Na hilo ni jukumu la serikali, wala isijaribu kukwepa, kwani Tanzania si nchi maskini. Nakerwa ninapowasikia viongozi wa nchi hii wakidai kuwa sisi ni nchi maskini. Juzi hapo nimemsikia waziri kwenye kipindi cha televisheni akitoa huo upupu kwamba Tanzania ni nchi maskini.

Nawashangaa wa-Tanzania waoendelea kuwapigia kura mbumbumbu wa aina hiyo, wasio na upeo kiuongozi, hawana fikra za kuijengea jamii ari ya maendeleo, badala yake wana upeo wa kudumaza na kukatisha tamaa. Nawashauri wajifunze kutoka kwa Rais Obama, ambaye katika mazingira yoyote yale, hawakatishi tamaa raia wake, bali anawahakikishai kuwa Taifa litapambana na litashinda, litafanikiwa. Lakini viongozi wa kwetu, kila kukicha ni vilio kuwa sisi ni nchi maskini!

Uwezo wa kuimarisha hali ya mahospitali yetu tunao, tukizingatia rasilimali nyingi tulizo nazo katika nchi hii. Pia, magonjwa mengi tunayopata hayahitaji daktari bingwa. Yanatibiwa vizuri, iwe ni Marekani au Tanzania. Tena yako magonjwa ambayo ni ya nchi kama zetu, ambayo wataalam wake ni hao madaktari wentu. Mifano ni malaria. Usidhani kuwa ukipata malaria, ukimbilie Marekani. Unaweza ukafia kule, kwa sababu sio madaktari wengi wanaijua malaria na namna ya kupambana nayo kama wanavyojua madaktari wa nchi kama Bongo. Hao madaktari wetu wanasomea magonjwa ya hapa kwetu. Ninavyofahamu, wanapoenda kusomea Ulaya, utakuwa wanasomea "tropical medicine" ambayo ndio uwanja wa mapambano katika nchi kama yetu.

Madaktari wetu wana ujuzi wa hali ya juu, utawakuta wako katika nchi mbali mbali. Ninawafahmu baadhi ambao wako Marekani, katika hospitali mbali mbali, kama vile Mayo Clinic, ambayo ni maarufu sana duniani. Yaani inashangaza kuwa ukienda hizo hospitali, unawakuta madaktari wengi wa kutoka Afrika, yaani madaktari kama hao wanaonyanyasika Tanzania kwa sababu ya viongozi wetu kutokuwa na upeo upasao.

Kwa hiyo, serikali yetu inachopaswa kufanya ni kuwekeza katika huduma za mahospitali yetu hapa hapa nchini. Iweke vifaa na miundombinu, ili madaktari wetu waweze kutumia ujuzi wao ipasavyo. Wakipata vifaa na miundombinu, na wakilipwa ipasavyo, watafanya yale yale ambayo baadhi ya watu nchini wanayatafuta India au ughaibuni. Tunapowapeleka watu nje eti kuchekiwa afya, wakati hizi ni taaluma za madaktari wetu pia, au tunawapeleka watu kutibiwa ugonjwa kama niliopata mimi wiki hii, ambao madaktari wetu wanajua namna ya kuutibu, ni kuwadhalilisha madaktari wetu, na mimi sitalilazia damu suala hilo.

Miezi michache iliyopita, nilipata fursa ya kumsikiliza Dr. Mark Jakobson, ambaye ni mwendeshaji wa hospitali ya Selian, Arusha. Alikuwa akitoa mhadhara Minnesota, kuhusu shughuli za hospitali hiyo. Siku moja nitaelezea katika hii blogu yangu. Jambo mmoja ninalokumbuka ni jinsi alivyowapigia debe madaktari wa ki-Tanzania wanaofanya naye kazi. Anatamani tu angekuwa na uwezo wa kulipa vizuri zaidi, ili wasipate vishawishi vya kwenda sehemu zingine.

Makala hii nimeiandika pia kwa sababu juzi hapo niliandika makala kuhusu mgomo wa madaktari nikaishutumu serikali kwa kudiriki kugombana na madaktari. Kwa hivi, nimejichukulia fursa hii kuelezea zaidi hisia zangu. Kati ya mambo niliyosema ni kuwa kisaikolojia, daktari anapaswa awe kazini akiwa amefurahi na moyo mkunjufu.

Huyu jamaa aliyenipima damu alitoa mchapo na tukacheka. Muuguzi au daktari anapokuwa na moyo mkunjufu na ucheshi, anapokuwa hana msongo wa mawazo, ni baraka kwa wagonjwa. Watafiti wa masuala ya saikolojia ya wagonjwa na matibabu wanathibitisha hilo. Ndio maana niliishutumu serikali kwa kutotambua umuhimu wa kuwaridhisha madaktari.

Taifa Msibani, Umoja Wetu na Utu Wetu Shakani

Jana asubuhi tarehe 10 Septemba 2011 Taifa liligubikwa na habari za kushtusha za maafa. Mwanzoni taarifa zilisambaa kwamba zaidi ya watu 2000 walikuwa kwenye Boti MV Spice Islander lililopinduka na kuzama mbele kidogo ya Nungwi likielekea kisiwani Pemba.

Habari hazikuwa za uhakika sana kwani vyombo vyetu vya habari na hasa Televeshini ya Taifa havikuwa vikitoa habari juu ya ajali hii. Habari zilisambaa kupitia mitandao ya intaneti na kufuatia uchu wa habari nilianzisha #ZanzibarBoatAccident katika mtandao wa twitter ili tuweze kufuatilia kwa karibu habari zote za ajali hii.

Habari zilisambaa kuwa Televisheni ya Taifa ilikuwa inaonyesha muziki wa Taarab na Baadaye vipindi vingine vya kawaida bila kutoa habari kwa Umma kuhusu msiba huu.

Watanzania waliokoa wakiwasiliana kupitia twitter na facebook walikuwa na hasira sana na kituo cha TBC1 kutoonyesha au kupasha habari juu ya ajali hii. Mpaka saa moja jioni kituo hiki cha Televisheni ya Taifa kilikuwa kinaonyesha watu wakicheza muziki wa Kongo, wanawake wakikatika viuno na hata ngoma za asili. 'Hallo, washa TBC1 uone Televisheni yenu inachoonyesha' yalikuwa ni maneno ya Mwakilishi Ismael Jussa Ladhu aliponipigia kuonyesha kusikitishwa kwake na kituo hiki.

Mpaka jioni tulikuwa tumepata taarifa kuwa Watanzania wenzetu 198 walikuwa wamepoteza maisha. 570 hivi walikuwa wameokolewa. Binafsi nilipotembelea Hospitali ya Mnazi Mmoja niliweza kumwona Binti aliyeitwa Leyla, akisoma darasa la Nne huko kisiwani Pemba. Leyla alipona, aliokolewa. Nilimwambia leyla atapona na kurudi Shuleni. Mpaka ninapoandika tumeambiwa kuwa Watanzania 240 wamegundulika kupoteza maisha na zaidi ya 600 wameokolewa. Napenda kuwapongeza sana waokoaji wetu, wavuvi wa kijiji cha Nungwi na vijiji jirani, wamiliki wa mahoteli waliojitolea vifaa vyao kwenda kuokoa watu na wanajeshi wetu wa majeshi ya ulinzi na usalama kwa kazi hii kubwa ya kuokoa. Kwa kweli tumeokoa watu wengi kuliko ilivyotarajiwa. Serikali ya Mapinduzi Zanzibar imejitahidi sana, ilitulia na kuonyesha uongozi dhabiti katika juhudi za uokoaji.

Rais wa Jamhuri ya Muungano wa Tanzania akafuta ziara yake huko Canada, ziara iliyokuwa imeandaliwa muda mrefu na kwa gharama. Lakini Rais aliona ni vema kupata hasara kuliko kuwa nje ya Nchi kipindi hiki. Hakutaka hata kumtuma Makamu wa Rais au Waziri Mkuu. Alifuta. Akatangaza siku 3 za kuomboleza. Hata hivyo waandaaji wa Miss Tanzania waliona hasara kubwa kuahirisha shindano la Urembo usiku huo. Wangepata hasara! Hata wafadhili wao Kampuni ya simu za mkononi ya vodacom ambao miongoni mwa waliofariki wamo wateja wa vodacom, waliendelea na shindano hili. Jumatatu, tarehe 12 Septemba 2011 ni siku ya khitma ya kuwaombea marehemu wetu. Nimeamua kutotumia simu ya ngu ya Voda kwa siku nzima, sina njia ya kuiadhibu Kampuni hii isipokuwa kugoma kutumia simu yao japo kwa siku moja kuonyesha kutoridhishwa kwangu na uamuzi wao wa kutojali msiba huu kwa Taifa.

Misiba huweka jamii pamoja. Msiba huu uliotokea Zanzibar ni jaribio kubwa kwa Umoja wa Taifa letu. Shirika la Utangazaji la Taifa limeshindwa jaribio hili la kudhihirisha Umoja wetu katika kipindi hiki. Nataraji wakubwa wa Shirika hili linaloendeshwa kwa kodi watawajibika au kuwajibishwa!

Jumla ya Majeruhi na marehemu sasa inafikia zaidi ya Abiria 800! Asubuhi ya jana 10 Septemba 2011 tuliambiwa na Serikali kwamba uwezo wa Boti la MV Spice Islander ilikuwa ni abiria 500 na wafanyakazi 12. Pia tumejulishwa kuwa boti hili lilikuwa la Mizigo na sio la Abiria. Natumai mara baada ya kumaliza maombolezo haya ya siku tatu Wakuu wa Mamlaka zinazohusika na usafiri wa Majini wa Serikali ya Mapinduzi Zanzibar watawajibika au kuwajibishwa. Mkuu wa Bandari ya Zanzibar Mustafa Aboud Jumbe na Waziri wa Miundombinu Hamad Masoud wanapaswa kufukuzwa kazi mara moja kama hawatawajibika wenyewe kwa kujiuzulu.

Kufukuzwa kwao kazi hakutarudisha ndugu zetu duniani bali itakuwa ni somo kuwa hatutarudia tena kufanya makosa yaliyoleteleza ajali kama hii.

Mwenyezimungu atupe subira wakati huu wa msiba mkubwa katika Taifa letu. Alaze roho za marehemu wetu mahala pema peponi na awape unafuu wa haraka majeruhi wetu.
